New School Immunization Requirements for 5th Through 10th Grade:

Important Information for Parents and Guardians

Beginning in August 2007:

• All students in 5th through 10th grade must show proof of Hepatitis B vaccination or proof of immunity.
• All students in 5th through 10th grade must show proof of Varicella (chicken pox) vaccination, proof of immunity or doctor's documentation of chicken pox disease with the month and year of infection.

Contact your child’s doctor to ensure he/she is immunized against Hepatitis B and Varicella.
